As part of our ongoing effort to improve our membership database, we ask for a few minutes of your time to send us the following information.
This information *will not* be made public as a part of our shul roster. Hebrew names will be made available to gabbaim, having your children's names and ages will allow us to send you youth group or educational information that is relevant to you. Yahrzeit information may be included in the weekly newsletter to encourage minyan attendance. Birthdays and anniversaries may be noted in the newsletter without mention of the specific date (or anyone's age!)
We realize that you may have included much of this information on your membership application. However, many applications have been archived, and in asking you to resubmit this information, we can be certain to have the most up-to-date details. Most of the form fields are optional and may be skipped if not applicable. If you are completing this form without having signed into the site first, it is important that you enter your complete name. Otherwise, your information may not be properly saved. Finally, if you are not already a member, do not complete this form. Instead, if you are interested in joining, please download, complete and submit the membership application in the sidebar to the left.
